Instructions | Use the Surgical Hx tab to record all of the patient's surgeries. We highly recommend that you complete the patient's surgical history prior to starting an encounter.
- Navigate to a patient's chart from the EHR Dashboard.
- Select the Surgical Hx tab.
- If the patient has had no surgeries, click No Surgical History.
- To add or edit surgeries, click Add/Edit History.
Add a Surgery
- To add a common surgery, click Common Surgeries.
- Click Yes or No to indicate whether the patient has had a surgery.
- Manually enter conditions that you do not see in the customized list.
- To specify an exact surgery, click Other Surgeries.
- Click a surgery type to display a panel of related surgeries.
- Click Yes or No to indicate whether the patient has had a surgery.
- Manually enter surgeries that you do not see in the customized list.
- The surgical history displays.
- To delete the negative history, click Delete in the Negative History section.
- To delete a surgery in the positive history, highlight the condition and click Delete.
indicates that this surgery cannot be displayed on the Patient Portal. To set portal privacy, click .- To add or edit the physician or any hospitalizations related to this surgery, click Edit Details.
- Enter the physicians and hospitalizations related to this surgery.
- Click Save.
Edit the Customized Surgeries Panels
This section covers editing the surgeries on, and adding surgeries to, the customized surgeries panels. Navigate to the Surgeries section by clicking Add/Edit History from the main Surgical History page.
- To edit the customized Common Surgeries panel, click Edit.
- On the surgical history edit screen, click
to delete a surgery. To confirm, click OK. - To reset the customizations to Common Surgeries for all providers, click Reset Category. To confirm, click OK.
- To delete the customized surgical history for the practice, click Delete. To confirm, click OK.
- To add a surgery to the Common Surgeries panel, click Edit.
- On the surgical history edit screen, enter a surgery in the SNOMED search box.
- Click a surgery that you want to add.
- Surgeries that display as selected are already on the Common Surgeries panel.
- To confirm the add, click OK.
- To edit the customized Other Surgeries panel, click a surgical category and then click Edit. You must click a category before the Edit button will display.
- On the surgical history edit screen, click a surgery category.
- To delete a medical condition, click
. To confirm, click OK. - To reset the customizations to the selected surgery category for all providers, click Reset Category. To confirm, click OK.
- To delete the customized surgical history for the practice, click Delete.
- To add a surgery to the Other Surgeries panel, click a surgery category and then click Edit.
- On the surgical history edit screen, enter a condition in the SNOMED search box.
- Click a condition that you want to add. This condition will be added to the condition category you selected.
- Conditions that display as selected are already on the Other Conditions panel.
- To confirm the add, click OK.
