Patients can send their medical records to email addresses from within the Send Patient Health Record page.
Patient Sends Medical Record to an Email Address
- From the Patient Account menu, click Send My Record.
- Fill in the form and click Submit.
- Select the check box to authorize the release of information to the recipient and click Submit.
Recipient Downloads the Patient's Medical Record
Recipients receive an email with a hyperlink to download the patient's medical record.
- Click Click here to download the medical record.
- Enter the patient's date of birth and click Download.
If the recipient enters the patient's date of birth wrong three times, the patient must send the medical record again.
