Schedule an Appointment and Register for the Practice

  1. Go to the provider's web page.
  2. Under Schedule an Appointment, click the desired appointment time.

You are redirected to the Patient Portal Schedule an Appointment page.

  1. Enter your name and select your date of birth and gender.
  2. Click Continue.

  1. Select whether you are the patient, the reason for visit, and your insurance.
  2. Click Continue.

  1. Enter your email and phone number, and create your username and password.
  2. Click Continue.

  1. Confirm the patient's information and select the I agree to the YourHealthFile User Agreement check box.
  2. Click Confirm.

  1. Log in to your account to complete the patient registration process.
