Resident Roster
Providers who provide care at nursing home facilities typically do not schedule visits for individual patients. They instead usually reserve a block of time to spend at the facility. To facilitate this workflow, Patient Tracker includes the Resident Roster view, which allows providers to navigate through the roster of patients who have been identified as residents of the nursing facility and their encounter status.
Resident Roster also includes the feature to create a new patient, allowing providers to quickly add a new patient as needed. This feature includes the option to indicate that the patient is a resident of a selected facility.
- Click the Resident Roster tab on Patient Tracker.
- The Resident Roster displays all patients who have been identified as residents of the selected location.
- Use the Search Roster feature to search for an existing patient. To clear the search results and display the roster, click Reset.
- The Encounter column displays each patient’s encounter status for the selected date of service.
- New: The encounter has not been started. To start a new encounter, click New.
- SOAP: An encounter is in progress. To navigate to the encounter summary preview, click SOAP.
- Signed Date: Signed encounters display the encounter’s signed date. To navigate to the encounter summary preview, click the signed date.
- Chart notes display beneath the patient’s name. Only the most recently created chart note displays. Chart notes are set to display by default. To hide them, select the Hide Chart Notes checkbox.
- To navigate to the patient’s chart, click the patient’s name.
Create a New Patient
- Click New Patient.
- To indicate that the patient is a resident of the selected practice location, select the Patient is a resident of this practice location checkbox. This will add the patient to the location roster and autopopulate the address and phone number fields with the location’s information.
- Click Save.
