Medical Hx
Use the Medical Hx tab to record all of the patient’s current medical conditions. We highly recommend that you complete the patient’s medical history before beginning an encounter.
- Navigate to a patient’s chart from the EHR Dashboard.
- Select the Medical Hx tab.
- To add or edit medical conditions, click Add/Edit History.
Add a Medical Condition
- To add a common condition, click Common Conditions.
- Click Yes or No to indicate whether the patient has the medical condition(s).
- Manually enter conditions that you do not see in the customized list.
- To save, click Back To Medical History List
- To specify an exact medical condition, click Other Conditions.
- Click a condition category to display a panel of related conditions.
- Click Yes or No to indicate whether the patient has the medical condition(s).
- Manually enter conditions that you do not see in the customized list.
- To save, click Back To Medical History List
- The negative and positive conditions display.
- To delete the negative history, click Delete in the Negative History section.
- To delete a condition in the positive history, highlight the condition and click Delete.
indicates that this condition cannot be displayed on the Patient Portal. To set portal privacy, click
.- To add or edit the physician or any hospitalizations related to this medical condition, click Edit Details.
- Enter the physician and hospitalizations related to this medical condition.
- Click Save.
Edit the Customized Conditions Panels
This section covers editing the medical conditions on, and adding medical conditions to, the customized conditions panels. Navigate to the Medical Conditions section by clicking Add/Edit History from the main Medical History page.
- To edit the customized Common Conditions panel, click Edit.
- On the medical history edit screen, click
to delete a medical condition. To confirm, click OK. - To reset the customizations to Common Conditions for all providers, click Reset Category. To confirm, click OK.
- To delete the customized medical history for the practice, click Delete. To confirm, click OK.
- To add a condition to the Common Conditions panel, click Edit.
- On the medical history edit screen, enter a condition in the SNOMED search box.
- Click a condition you want to add.
- Conditions that display as selected are already on the Common Conditions panel.
- To confirm the add, click OK.
- To edit the customized Other Conditions panel, click a condition category and then click Edit. You must click a category before the Edit button will display.
- On the medical history edit screen, click a condition category.
- To delete a medical condition, click
. To confirm, click OK. - To reset the customizations to the selected condition category for all providers, click Reset Category. To confirm, click OK.
- To delete the customized medical history for the practice, click Delete. To confirm, click OK.
- To add a condition to the Other Conditions panel, click a condition category and then click Edit.
- On the medical history edit screen, enter a condition in the SNOMED search box.
- Click a condition you want to add. This condition will be added to the condition category you selected.
- Conditions that display as selected are already on the Other Conditions panel.
- To confirm the add, click OK.
