Improvement Activities is one of the four MIPS performance categories. EHRs were not certified for Improvement Activities, thus it is the practice's responsibility to provide proof of completion. To assist, NextGen® Office has features to help you comply with the category's measure requirements.
Document Types
Policy and Procedures
You should upload policy and procedures for each selected Improvement Activity. Document in detail which actions are performed to comply with the measure requirements. Explain the features that are used within the NextGen Office application, and review the day-to-day procedures that staff are required to perform.
Care Coordination Agreements
This feature helps pass the following recommended measures:
- Care Coordination Agreements That Promote Improvements in Patient Tracking across Settings
- Implementation of Use of Specialist Reports Back to Referring Clinician or Group to Close Referral Loop
Establish care coordination agreements with frequently used consultants that set expectations for the documented flow of information and MIPS eligible clinicians or MIPS group expectations between settings. Provide patients with information that consistently sets their expectations with the care coordination agreements.
Community Resources
Uploading a document as a "Community Resources" type makes it accessible to all patients in the patient portal under Documents > Community Resources. To avoid accidental disclosure of Protected Health Information (PHI), please do not upload documents containing PHI using this document type.
This feature helps pass the Practice Improvements That Engage Community Resources to Support Patient Health Goals measure. Develop pathways to neighborhood/community-based resources to support patient health goals that could include one or more of the following:
- Maintain formal (referral) links to community-based chronic disease self-management support programs, exercise programs and other wellness resources with the potential for bidirectional flow of information; and/or
- Provide a guide to available community resources.
Upload Documents
- Select ACTION and select Upload New Document.
- Select the Document Date.
- Type the Document Name.
- Select the Document Type.
- To search and open the document, select Browse.
- Select Upload.
Manage Documents
Use the Manage Documents feature to add, edit, and delete documents.
Open Manage Documents
Select ACTION and select Manage Documents.
Add Documents
- Select ADD DOCUMENT.
- Upload the document.
Edit Documents
- Select ACTION and select Edit Document.
The Upload Document window opens.
- Make your edits and/or upload a different file.
- Select Upload.
Delete Documents
- Select ACTION and select Delete Document.
- Select Confirm.
- Select Close.
