1. Are there training guides on how to set up panels for ICD-10?

Yes. Training guides are already posted to the NextGen Office Success Community.

2. Can NextGen Office staff help me choose codes for my bills?

No. Our staff is not permitted to suggest codes for a claim. They are not certified coding experts.

3. Should I obtain training on how to use the ICD-10 coding system?

Yes. Providers and billing staff are expected to arrange outside training specific to their specialty before October 1, 2015.

4. How specific does my code need to be?

For one year, Medicare will pay all billable codes within the family of codes (first three digits) that apply to the patient’s clinical presentation. Beginning in October 2016, Medicare will require a greater level of specificity. Other payers have not committed to how specific the code needs to be in the first year.

5. What ICD coding system should I use for dates of service before October 1, 2015?

The ICD-9 system – even for claims that are resubmitted with a DOS before October 1 that are resubmitted after October 1.

6. What ICD coding system should I use for dates of service on, and after, October 1, 2015?

The ICD-10 system

7. Can I have ICD-9 and ICD-10 codes on the same claim?

No.

8. What happens if I have a claim with a date of service From Date before October 1, 2015 that spans a time frame past October 1, 2015?

File two claims. The first claim is for dates of service before October 1 and should be coded in ICD-9. The second claim should be filed for dates of service on, and after, October 1 with ICD-10 codes.

9. How can I practice how to code in ICD-10?

Work with your biller to examine your commonly used ICD-9 codes and map them to the ICD-10 codes to prove that you know how to bill your commonly used codes. Practicing before the October 1 deadline is imperative and recommended by industry experts. Your practice’s coding expert should be familiar with the various scenarios that relate to your specialty.

10. How does SNOMED relate to ICD-10?

SNOMED is the required format for Problem List not diagnosis coding for billing. There is a mapping tool in the Assessment (Diagnosis Coding) module in the EHR that assists in mapping SNOMED Problem List codes to ICD-10 codes.

11. Can the NextGen Office create code panels per practice by the ICD-9 codes each practice bills?

The system has tools to assist, but the practice is required to choose which codes they want to include. Since there is a one-to-many relationship between the ICD-9 and ICD-10 code systems, only your practice knows which codes to include. Setting up new ICD-10 panels based on your current ICD-9 panels should be a simple task.

12. Do we need to attend every ICD-10 Boot Camp?

No. Much of it will be repetitious, but you should attend until you completely understand the covered topics. Each session may cover different questions from other attendees that may be useful.

13. If you have your doctor’s own code set with the Dx used most, will it match up to the ICD-10 as well?

Since you already know your favorite ICD-9 codes, map them to the ICD-10 codes using the tools built into the software and covered in our webinars and training documents.

14. Does anything change with the CPT procedure coding system or the modifiers used?

The ICD-10 rule does not impact the CPT procedure coding system.

15. Can we start using ICD-10 before October 1 to get practice before the deadline?

You can create practice claims but ICD-10 codes cannot be submitted to the payers until October 1, 2015. The payers will not be adjudicating claims prior to the deadline. We have tested end to end with certain payers to make sure our claim format can be accepted into the payer’s adjudication system.

16. If my workers’ comp payer does not accept ICD-10 for DOS on, or after, October 1, what should I do?

Print and mail a paper claim. No ICD-9 codes for DOS after October 1 can be sent electronically.

17. In the assessment portion of the encounter, after mapping the ICD-10 for that encounter, do I have to do it for each future encounter or does it save to the problem list?

It will save to the Patient History button. Remember the Problem List should be coded in SNOMED not ICD-10.

18. Test claims, per CMS, is allowing test claims if “T" is indicated in the interchange control structure (ISA) field, for example. Are we able to do this? If so, how?

No. All payer testing is performed by NextGen Office. This type of testing is only for end to end and will not result in an adjudicated claim.

19. In preparing new patient charts in NextGen Office, if we map a condition in the problem list to ICD-10, and then in a few weeks after October 1, if we are documenting an encounter and pull from the problem list while selecting a diagnosis for the assessment portion of that encounter, will NextGen Office display the ICD-10 code that previously mapped?

No, but the same SNOMED-to-ICD-10 coding tool is available on the Assessment (Diagnosis Coding) page.

20. If there's an ankle sprain, but I'm not sure which ligament is involved, which ICD-10 should I use?

There are codes that have an “unspecified” ligament. You would use one of those.

21. Can I set up my ICD-10 code superbills or folders “on-the-fly?”

No. You should not be spending time during encounters on creating superbills. Just like with ICD-9, they should be set up in advance.

22. In preparation for October 1, do you suggest we map codes from ICD-9 to ICD-10 in encounters now? And, if so, won’t the ICD-10 and ICD-9 codes and descriptions show up in the encounter?

Yes. You can map the codes now in the Assessment area of the SOAP note. When mapping ICD-10 codes from this area, the code will not be added to the bill and the codes will be available for the future when you are ready to use ICD-10.

23. How do I search ICD-10 on the PM side?

The ICD-10 feature you see in the EHR will be available in the PM Charge Entry screen very soon. The functionality is the same.

24. When I map an ICD-9 code in the EHR in the patient’s chart, is this for all patients or one patient?

This is for one patient. Each patient’s scenario may differ and will require the practice to choose the appropriate code for each clinical scenario.

25. In the PM, I have set up a billing slip for ICD-10, how do I switch on October 1?

If your practice uses billing slips on the PM side, this can be set up by going to Admin > Billing Slips. On October 1, you may assign the billing slip to each location by going to Admin > Calendar Resources and then editing the desired resource and assigning the superbill based on location.

26. Can I use denial defender with the new ICD-10 codes?

Yes. The denial defender tool will have the same functionality and will accommodate the new ICD-10 codes.

27. Can I use ICD-10-PCS codes?

No. You must only use ICD-10-CM codes. ICD-10-PCS is a procedural classification system and a separate coding system from ICD-10-CM. ICD-10-PCS codes can only be used for inpatient, hospital settings. Learn more about the difference between the two coding systems: ICD-10-CM vs. ICD-10-PCS.
