Issue

NextGen Healthcare has identified an issue with NextGen® Office EHR that occurs when the Patient is born premature checkbox is selected while vitals are documented in the Vital Signs top tab and within an encounter. As a result, some of the selected vital sign data points do not appear on a growth chart.

Steps to Recreate the Issue

  1. Create or open a patient chart with the date of birth 5/6/2024.
  2. Open the chart-level Vitals tab.
  3. Select the Patient was born premature checkbox, and enter 35 for Weeks.
  4. For Date, enter 5/6/24.
  5. Enter 5 lb 11 oz for weight, and then select Save.
  6. Select the Encounter tab, and then change the date to 5/13/24.
  7. From the SOAP menu, select O, and then select Vitals.
  8. Enter 5/13/24 as the date, and then enter 5 lbs 3 oz for weight.
  9. Enter 18.5 inches for height, and then select Save.
  10. Select the checkboxes for both 5/6/24 and 5/13/24, and then select Growth Chart.

The chart should display two weight data points but displays only one.

Actions Required

Until this issue is fixed, users can enter historical vitals (including birth vitals) in an encounter instead of via the chart level/top tab.

Status

This issue will be fixed in a future release.
