Proxy Is Not Recorded as Reviewer of Completed Patient Registration Documents

Issue
In NextGen® Office Patient Portal, when the proxy completes the patient’s registration documents, the patient’s name is recorded as the reviewer in EHR and not the proxy.
Steps to Reproduce
- In EHR, grant proxy access for a patient chart.
- Log in to the Patient Portal as the proxy.
- Complete the registration documents.
Editable PDF documents will pull the proxy’s signature when saved at the end of registration.
- In EHR, from the patient’s chart, click Administrative.
- From the Documents section, click a document’s Title to download a patient registration document (an editable PDF).
- Scroll to the bottom of the document and view the name next to Electronically Signed and Reviewed By.
The patient’s name appears instead of the proxy’s name.
Action Required
Until this issue is fixed, inform proxies not to complete the intake information on behalf of the patient. If the proxy is one of the patient’s parents, the NextGen Office team suggests completing patient registration forms in the office on paper and then scanning the documents into the record.
