Negative Surgical History Added in the Patient Portal Does Not Match in EHR

Issue

When patients add their surgical history in the NextGen® Office Patient Portal, it shows as pending data in EHR for providers to accept before it is added to the chart. In this internally reported issue, negative surgical history data that was entered in the Patient Portal does not match the pending data in the patient's chart. If one negative history entry is added, the Negative History section shows in the chart with no data. If two or more negative entries are added, the Negative History section shows in the chart with no data and commas (the number of commas depends on the number of entries). If one positive and one negative surgical history entries are added, the positive history entry shows in both the Negative History and Positive History sections. Other scenarios may occur depending on the number of positive and negative history entries added. The expected behavior is that each negative history entry added in the Patient Portal shows in the chart on a separate line with its own check box.

Steps to Reproduce

  1. In the Patient Portal, the patient clicks Review Medical Record and selects Medical History.
  2. The patient clicks the Surgical History tab.
  3. They add their surgical history.
    • Scenario 1: Add one negative surgical history entry. 
    • Scenario 2: Add two or more negative surgical history entries. 
    • Scenario 3: Add one negative and one positive surgical history entries. We selected Yes for Aortic Aneurysm and No for Mastectomy.
  4. In EHR, open the patient's chart.
  5. Click the Surgical Hx tab.
  • Scenario 1: The Negative History section shows with no data.

  • Scenario 2: The Negative History section appears with no data and one comma since there are two entries.

  • Scenario 3: Aortic aneurysm shows under both Negative History and Positive History.

Note: Other results may occur depending on how many positive and negative history entries are added together.

Action Required

We recommend that providers verify the patient's negative surgical history before accepting their additions in the chart.

This issue was fixed on October 7, 2021.
