Encounter C-CDA Is Exporting the Patient’s Current Medical Information

Issue
The C-CDA is not exporting the medical information that was present at the time of the encounter; instead, the C-CDA is exporting the most up-to-date medical information. If new information has been updated in the chart since the encounter, the C-CDA will reflect this. There are circumstances where this may cause confusion by the sender or recipient of the C-CDA. This issue applies to following pages and sections: Allergies, Family History, Immunizations, Medications, Problems, Social History, and Vital Signs.
Action Required
If you are sending an encounter C-CDA, but the patient has information added subsequent to that encounter, you should also send a clinical summary PDF. The PDF will accurately reflect what was recorded during the encounter.
Status
Further investigation revealed that the C-CDA exporting current records is intentional. C-CDAs should and will always be generated with the most current data since the purpose of a C-CDA is to transfer information to an external system and the assumption is that the most current information is what is desired.
