Issue

In NextGen® Office EHR, data can be imported into signed/finalized encounters using Grand Central without reopening the signed encounter, making it appear as though the newly imported data has been signed off by the provider.

Steps to Reproduce

Use Grand Central to Import Data into a Locked and Signed Encounter

  1. In the patient chart, create a new encounter (Encounter A).
  2. Add data.
  3. Sign Encounter A.
  4. Create another new encounter (Encounter B).
  5. Add data.
  6. Sign Encounter B.
  7. Click the Encounters tab and view the encounter list.
  8. Under the Finalized column, click SOAP for Encounter A.
The Encounter is Locked. Signed encounters can only be re-opened by the Signer or Cosigner alert appears.
  1. Click Grand Central.
  2. Under Patient Blueprints, click Prior Encounter.
  3. Click to select Encounter B and then click Import.
  4. Click blue Encounter button to go to the Encounter Summary Preview page.

Note that the data for Encounter A remains unchanged. View the newly added data in the signed encounter.

Issue 1

  1. Click Clinical Summary.
  2. Click Export PDF.
Note that the Encounter A Export to PDF data has changed.

Issue 2

  1. Click the Encounters tab and create a new encounter (Encounter C).
  2. Click Grand Central.
  3. Under Patient Blueprints, click Prior Encounter.
  4. Click to select Encounter A and then click Import.
  5. Click the blue Encounter button to go to the Encounter Summary Preview page.
Note that Encounter B data appears when Encounter A was imported.

Issue 3

  1. Click the Encounters tab and view the encounter list.
  2. Under the Finalized column, click SOAP for Encounter A.
  3. The Encounter is Locked. Signed encounters can only be re-opened by the Signer or Cosigner alert appears. 
  4. Click Re-open the Encounter and then click OK to confirm reopening the encounter.
Note that Encounter B data is now present in the Encounter Summary Preview for Encounter A. Encounter A’s data will be overridden by Encounter B data in the sections where Encounter B data is present.

Action Required

Do not use Grand Central to add more data to a signed and locked encounter. Even though Grand Central is enabled, do not use this feature after the encounter has been signed.

Status

This issue was fixed on July 25, 2019.
