Abnormal Phrases in Physical Exams May Be Removed When Edited Twice

Issue

In NextGen® Office EHR, when a user saves an abnormal phrase in a physical exam and edits the phrase twice, the abnormal phrase may be removed. The abnormal phrase may be removed regardless of whether changes are made to the phrase.

Steps to Reproduce

  1. Open an encounter that has a documented physical exam.  
  2. Click O on the SOAP menu and select Physical Exam.
  3. Click a body system button that has a documented physical exam.
  4. Click default red phrase icon to change a normal phrase to abnormal.
  5. Edit the phrase.
  6. Click Save.
The changes save.
  1. Click the same body system button as in step 3.
  2. Click Save.

The phrase resets and the changes disappear.

Action Required

For every subsequent edit to the default phrase, copy and paste the existing content before editing the phrase. Edit the information and then save.

This issue was fixed on April 6, 2023.
